Assessment Process

Our Assessment process entails a meeting where all relevant information is gathered from the individual, the family or carer and the multidisciplinary team. The assessment consists all necessary information such as medical history, list of current medication, social history and daily living needs. It is very important for us that the family (and carer where applicable) is present during an assessment and active in all stages of care planning for their loved ones.

The assessment serves many different purposes. Some of these purposes are outlined below:

  • Detailed analysis about an individual’s needs
  • Understanding of the individual’s care needs before care is provided.
  • Familiarity – the potential client would meet at least one member of the staff team before we commence support.

Assessment could be either face to face or over the telephone. During the face to face appointment, our assessment officer will meet with the intended client to know their care needs.

We recruit via several reliable methods. We use websites, target groups, job fairs and focused advertising. 

We put interested applicants through a thorough interview process that includes both individual and group settings with hands on practical classes. 

Candidates are assessed through a thorough interview process, references, police reports wnd forms of Identification are obtained. 

We ensure our carers receive sufficient training before they assigned to client support.

Frequency of visits will vary with each client. Visits will be determined based on the client’s condition and basic needs.
